此件玉牌为和阗白玉制成，玉质上佳，带有褐色沁。器体呈长方形片状，整体雕作一只匍匐状的虎，虎首向下，粗眉怒目，鼻似卷云，身体扁阔，四足匍匐于玉牌背面，尾巴作仿古装饰翻翘至背部，身体装饰勾连云纹，造型威严生动，气势不凡。背面正中打双阴线方格，内刻篆体“五千石”三字。整体形制古雅，应为当时使用者身份地位的象征，同时具有辟邪之意，十分独特。
